  • Balanced Electrolyte Solution: Lactated Ringer's (LR) is a balanced electrolyte solution designed to closely resemble the electrolyte composition of human plasma. Each 1000 mL contains:

    • Sodium (Na+): 130 mEq

    • Potassium (K+): 4 mEq

    • Calcium (Ca2+): 3 mEq

    • Chloride (Cl): 109 mEq

    • Lactate (C3H5O3): 28 mEq (which is metabolized into bicarbonate in the liver)

  • Isotonic: With an osmolarity of approximately 273 mOsmol/L, LR is isotonic with human blood, meaning it helps to expand the extracellular fluid volume without causing significant fluid shifts into or out of cells.

  • Source of Bicarbonate (via Lactate Metabolism): The lactate component is metabolized by the liver to bicarbonate, which provides an alkalinizing effect, making it useful in situations of mild acidosis.

  • Sterile and Nonpyrogenic: Ensures the solution is free from microorganisms and fever-producing substances.

  • Flexible Container (e.g., Viaflex® plastic bag): Typically packaged in a flexible plastic container (such as a PVC/DEHP bag) for ease of use, transport, and waste reduction.

  • Single-Dose Container: Designed for single use; any unused portion should be discarded to maintain sterility.

  • Volume: 1000 mL, a common volume for intravenous fluid administration.

  • Prescription Drug: Requires authorization from a medical director or pharmacist-in-charge for purchase.

Uses:

  • Fluid Resuscitation: The primary use is for rapid and effective replacement of fluid losses in conditions such as:

    • Trauma and hemorrhage (blood loss)
